Lloyds life insurance Amazon offer

I could have sworn there was an offer back on Oct/Nov for an Amazon voucher if you took out life insurance via Lloyds bank. I think it was Lloyds. Anyhow I took out the policy but seen no sign of the gift card yet.
Did anyone lese sign up to this?
