Power of Attorney - who gets told, and what?

My partner and 2 siblings recently were appointed Attorneys of their mother's Financial and Health. Mother is now diagnosed as having Dementia. One sibling has mum's bank statements and has noticed a very recent payment (after the LPA was granted) to the office of public guardian which looks like she has changed one of her powers of attorney.
my question is - if she has changed the named attorneys would anyone be informed they are no longer attorneys?

OPG would send a letter to who had been removed to advise them that they had ceased to be a POA as well as a letter to the donor

I'm not sure that it is possible to change an attorney, I think that the original LPA will have to be cancelled and the whole LPA re-done. How much was the payment for?

Yes as I thought, you can't just "add" another attorney to an existing LPA
