Covid 19 recovery certificate

I'm travelling to USA in February 22, but have had covid. They need a recovery certificate so I don't have to do a lateral flow, anyone had one of these forms or any help would be appreciated?

The NHS do not provide the covid recovery certificate required by the USA . You will need to find a private Dr who provides this service- you will have to pay .
