My vaccine certificate does not show my middle names, but my passport does.
Most sites state that the certificate must match the full passport name.
My surgery says that NHS does not use middle names.
Has anyone with middle names successfully travelled abroad, USA in particular, using a vaccine certificate without their middle names?

Yes.
Went to Spain, I just filled in their online form and left my middle name out.
Someone else on here did the same.
Cant imagine the US is any different, the names that matter match after all.1 -
