Seems pretty obvious that the ideal solution is a comprehensive set of integrated systems, accurately and carefully specified, designed, written and tested ahead of staged live implementation, but equally obviously, circumstances have mitigated against that, with ever-changing requirements and timescales.  I'm not necessarily defending where we are (and, as above, this is all speculative anyway) but just highlighting that a degree of pragmatism has to be expected, i.e. "don't let the perfect be the enemy of the good" - there are many aspects of testing that are essentially reliant on trust rather than rigorous preventive controls that are more desirable than practical.

Anyway, this is all getting further away from the thread topic so probably best to leave it there....
