It asked me for an email address after I chose the option that I don't bulk buy, and it claimed the survey was for bulk buyers, but enter your email address anyway to be entered into the prize draw.

Good spot. If you say no to question 9, you get to enter your email address. If you've already completed the survey you can't do it again in Chrome unless you open the link in an incognito window - right click and select 'Open link in incognito window'
