    So if FOS are not the right people, who should I alert to the fact their systems/procedures have issues?
    Perhaps I wasn't clear enough - I wasn't disputing that NS&I have issues but was simply highlighting that FOS have no executive authority to actually do anything about them, in that their remit only extends to reviewing how they handled your complaint, i.e. they're an ombudsman service rather than a regulator.

    I'm lucky, I have an alternative account that allows me to withdraw money (for as long as my wife is alive as it is a joint account) 

    If this is a standard joint account, then there should be nothing to prevent you from accessing the account in the event that your wife should predecease you.

    Nothing either to prevent your standing orders/direct debits from continuing to be paid.

    It is for this reason that many married couples choose to have a joint account for such payments as electricity/water/insurance etc.

    You would supply the bank with a copy of the death certificate and the name of the deceased would then be removed from the account making it a sole account.
