Android call blocking off but withheld number still not getting through

Yesterday I missed an important NHS consultation: no phone call came but they say they called. They usually use number withheld - my Android setting to ‘Block calls from unidentified callers’ has always been turned off. My network provider says they don’t block any calls. I tried calling from another phone using 141 and it came through ok as Unknown.
I have taken calls from the same source before: the two things that have changed are my network provider and my phone (iPhone to Android 11). The phone manufacturer (Xiaomi) assure me they have no hidden apps that would override call block settings.
I don’t know what else I can do - as things stand the same thing will only happen again with any new appointment. They have the correct number and used it successfully for sms reminders. Is there some setting I’ve overlooked?

There is the possibility that they didn't actually phone you or something else went wrong from their end.

You haven't told us who your network provider is. If it's Virgin Mobile then they had an issue that started yesterday (and has been fixed today) where incoming calls got a "Sorry, the person you are calling is unavailable" message, and some, but not all, text messages were held up. Everything outgoing was fine.

Yes, there are no missed calls. The Network provider is Lebara, they have told me that "Lebara does not block any calls but if you are unable to receive calls from NHS then I will raise the request to the team to check and get it fixed for you". Which was odd wording.
The hospital has so far checked the number with me, which was correct. They haven't said anything more - but I imagine the number is digitally dialled from their records rather than entered by hand.0 -
