    They very rarely contact anyone for any evidence so when filling in the work capability assessment form you need to make sure you send the evidence to support his claim. Don't assume anything. The majority of people have assessments, paper based assessments are rare but definitely possible.
    As advised in my earlier comment, if he's not mentally capabile of looking after his own benefit claim then you can become his appointee. This way you will be responsible for everything, which will include answering the questions if an assessment is needed, although he will need to attend with you (if he requires a face to face assessment) although telephone assessments are still being completed.

    He should automatically be treated as having LCW because of being in rehab, but they will need further information to determine whether he has LCWRA (and therefore the extra element).  As already mentioned that can do a paper-based decision if the form is filled in well and you send supporting evidence; I would suggest also including information about his prognosis so they don't erroneously decide he should be for fit work in some ridiculously short time frame - he would have to be reassessed before they could make that decision, but may as well give them good reason to leave him alone for as long as possible.

    I would strongly suggest asking for help for the form.  It will help if you have an understanding of the descriptors yourself and how they are assessed but it can be a lot to take in especially when you have other important things on your mind, so having help from people who already understand it would probably be best.  The same will go for PIP when he comes home and can apply.
