Redundancy Advice Please!

Hi, so I found out last week that I was being made redundant as the buisness I have worked at for the past 6 years is going into liquidation. I expected this news as we where one of the first places closed due to covid and one of the last places that where allowed to open back up (in our case we didn't though).
The agency assigned to the insolvency has sent me through my redundancy letter with the link to claim from the government. My issue is I have been struggling with this form and it's giving me a bit of a headache. For example my shifts where varies before lockdown as I was 0 hours but had decent shifts, have been informed by agency dealing with the process to select that I did have a fixed salary and fixed rate because I have been furloughed over a year. Ok that's fine.
The next big issue for me and where I stopped my form and will have to go back and start again because it was giving me a headache, is it asks you to input average hours worked for the past 12 week..... this is where I am stumped. The person I have been emailing said to try and input 0 due to furlough... didn't let me move forward, so I tried to input just 0, wouldn't let me move forward. He has suggested putting in my average hours from before I was put on furlough back in 2020 but I don't know if that's what I should be doing because it says the past 12 weeks.
This form isn't very furlough friendly at all and is giving me major anxiety and headaches. Was just wondering if anyone else has struggled or figured out how to do this. I just don't want to make a mistake and mess up my claim for money I'm due. I dread to think what even comes next on this form. So much to deal with when have had little to no guidance 😕

'The last 12 weeks' is to ascertain your normal pay. Given that your normal pay routine hasn't happened since last year, I'd be minded to enter the figures that match the last 12 weeks of normal working, with a note somewhere if there's a space available, to clarify this.3
