Urgent: email N180 form to highview/DCB?

Hello, 

I can’t locate my N149A Form right now to know who to send the N180 form (deadline today!) to as well as the CCBC email address. 
I had a letter from DCB legal with info@dcblegal.co.uk is this the one to use? 

Thanks in advance to anyone who can help, in a bit of a panic right now!
