When? Whenever the government considers it necessary, rightly or wrongly
Which? supposedly depends on the state of infection in the country concerned

Everyone complained when the UK gave 4 days notice of a change from green to amber for Portugal but Germany has now imposed a 14-day quarantine for arrivals from Portugal so we may have been right.

Right now I fear the next change will be Majorca which stupidly allowed thousands of unvaccinated students to arrive and spread it around the island. In a desperate attempt to control it, they quarantined positive students in a very decent hotel but the courts ordered them to let them out, at last count they had spread it widely in Madrid and Andalucia when they went home

Nothing to "work out", you can just speculate.

I believe an update to the house of commons is due this week but that could be as simple as saying we're still thinking.

Its been suggested that there may be changes in rules for those entering the UK from Amber countries who are fully vaccinated with a view of "august" as the implementation time but nothing more definitive than that.

Suggestion that from 19 July, fully vaccinated people won’t have to isolate on return from amber countries. They will still need an antigen test to travel back to the UK and a full PCR on or before day 2.
