PPI not asked for but was added

My Capital one card was set up about three or four years ago.
When I signed the application form I ticked the box asking to not have PPI on this credit card. Last November they started charging me for PPI and I have never signed a document to ask for this.
Is this legal for them to do this and is it possible to reclaim all that I've paid?
Is there a ready made form on this site?
