Probate - Insurance policies Question

This is the last section I need to fill in...
My parents both passed within 10 days of each other earlier on in the year, Mam first then Dad. We just received 3 Insurance cheques, 2 for Mams policies and the other for Dad.
"Policies that are paying out to..."
Do I only need to list the one in Dads name or all 3?
