Road Tax Issue!

Hello there.
Couldn't really find the correct category to out this under so might as well!
I am wondering if someone could tell me where I went wrong with this or possible shed some light on it. 
My vehicle has been taxed since I've bought it (sometime in 2019) and I've been making regular payments to DVLA via DD. In October 2020 I requested a change of name on the logbook (change to my own company's name, same address) upon which they requested further information in April 2021 via email. I sent the email and then heard nothing. Did not receive a new logbook. All this while payments were being taken from my bank account and I considered myself perfectly legal and happily road-taxed.
This morning I walked out to find my vehicle clamped because apparently it was untaxed since October 2020! And even in their system right now, it shows up as untaxed even though my bank statements clearly show alongwith my car reg number, otherwise. Tried to get in touch with someone but it seems their system is in the stone ages. I received absolutely no notification, warning or reminder that my vehicle was untaxed. And if it were, why were the payments still being taken? I just don't get it. 
Did I go wrong somewhere? Please can someone explain what to do now. I have already payed the £100 unclamping fee.
