OP, no-one can second guess whether the other school will find out about the disciplinary or how much they will care if they do.
It may be that your existing manager will give you a glowing reference as they also find it awkward working with you and would like you gone.
It may be that any reference will be a very rigid, formulaic one answering specific questions only. If that's the case, it may well mention that you had a recent disciplinary, (assuming, it's not clear from your post, that there was some form of sanction against you) and may also mention your sickness record which might be off-putting.
If I were you, I would focus on trying get back to work and doing the best you can , while continuing to look for new jobs elsewhere.
