Your MCOL claim history will indicate when an n180 DQ has been posted to you , at which point you email your completed DQ to the ccbcaq email address
You should reply to the above letter, but not answer their questions in detail

1: How would i know as this was nearly five and half years ago. A question I will certainly ask the judge if he remembers what he was doing in that time period.
Providing insurance information is irrelevant unless you wish to pick a name out of a hat.
2: Again, nearly five and half years ago, such information is unknown
3: Read 1 & 2 above
I will of course show the above reply in court.
I will be requesting the court to question the validity of the extra charges you have applied to which you have not provided any information or your legal authority.
I am fully aware that a statement of truth is required and am concerned that as you have not explained your legal authority to add unknown amounts, that you have signed a statement of truth that reflects this.
This will be brought to the attention of the judge

I have received a letter today (dated 27th Oct 2021) allocating me a telephone hearing. Unfortunately, since I submitted the court paperwork (back in July) we have booked flights for a holiday over Christmas and I won't be able to attend the hearing set for 23rd Dec 2021. I intend to provide evidence of my flight details to the court, but I think I'm right in saying that they won't allocate an alternative date and that the hearing will take place without me.
I will make sure I refer to the thread on telephone hearings for what I need to submit prior to the hearing, but I wondered if you had any additional guidance in this case? Thank you!0 -
