Renunciation form

The executors of my mum's will live in New Zealand and they've agreed to renounce, so I've downloaded form PA15 from gov.uk for them to fill in and sign. It's a very simple form, but there's no field for country - ie, it assumes people will be entering UK addresses, not international ones. Has anyone used this form outside the UK before? Perhaps they can just put "Auckland, New Zealand" in the "city or town" field? Or should I draft a more suitable letter?
