Stair lift problem

I'm desperate for advice and can't find anything online so I hope it's ok posting here.
I've just moved into a new build property. The housing company applied for a grant for a stair lift to be fitted, a community OT was supposed to come out to the property to assess which type of stair lift I would need and authorise it and send her report to the housing company. I have never met this community OT or spoken to her. She took it upon herself to order a straight stair lift which was fitted (I thought it was a curved one which would take me to the top landing) it was supposed to be a curved one. Instead it stops just short of the first landing on the top step and swivels around onto the landing, this is quite unnerving. I then have to struggle up to the one step onto the top landing. The housing company can't alter it as it has to be authorised by this community OT who refuses to come out to see the property. I have also emailed her to ask for a copy of her report for my reference and she hasn't answered my emails or sent her original report or contacted me in any way. The housing company also refuse to send me a copy of her report. I also don't know who authorised the grant for the stair lift. The community OT is contacted through my local council. I have copies of the reports from my last 2 properties I've lived in. The 1st was a private rental and the 2nd was council so I know I can and should have a copy. Can anyone help please.     I'd suggest following the complaints procedure for the department that the OT falls under, which I'm guessing is adult care at your local council?

    You could also try local MP or district councillor.

    You could also raise a formal complaint with the housing company, because they have allowed this unsafe stairlift to be fitted.   What reason are they giving for not releasing the report?

    First step would be to Ask to speak to the team lead (line manager of the ot).  
    Have you considered sending a photo so they will see what you are talking about?
