Enforcement of magistrates order against landlord to reinstate heating and hot water

My landlord cut off my heating and hot water and I obtained an order from a Magistrates Court to reinstate it. Despite the order, they have not reinstated it. I wrote to the court asking them to enforce the order, but they have told her me to apply using the proper form. I do not know what form to use and the court will not tell me, advising me to take legal advice. The Council has been of no use and I have rung several firms of solicitors who aren't able to help me. I tried to research this issue, but came up with a blank! Nobody seems to know which is the right form to use to enforce the magistrates order.
