Completion of APHA form ABPR04 - making dog treats from home.

Does anyone have any knowledge on how to complete form ABPR04 (Application and Inspection form for approval of a Pet Food Plant)?  The form is blowing my mind and the Microbiological Sampling questionnaire has stumped me.  Any advice on this, especially where I need to get samples tested at raw, during process and at storage stage?  Also,do I need to be specific about what products I am making at this point?  I will be making meat based and meat free, but may not make meat based every week, so how can I test weekly?  Am I overthinking all this?  Any help is very much appreciated.  Many Thanks
