UC50 not received

Hi guys,

I returned a UC50 questionnaire back to the centre for health and disability assessments on the 24/3/21, over 2 weeks ago.

I phoned earlier today, and was told it's not been received.

As the UC50 was returned 1st class in a prepaid envelope, I would expect it to be received within 2 > 7 days worst case.

    The UC50 is to establish whether or not you have Limited capability for Work or Limited capability for Work and Work Related Activity. Even if the UC50 is not returned your UC would not stop, it would simply mean that you are treated as fit for work.

    When you spoke to them did you ask them to note that it was posted out on 24th? 

    Did you keep a copy? If so you could print it out again and send another copy if necessary,

    Hi Calcotti,

    I did indicate that my UC50 was returned on the 24th of March, and the response was that it should be there, however, phone back next week to check again.

    I have submitted a fit note on my UC journal several weeks ago, and that expires on the 26th of April for what it's worth.

    The rep' I spoke to at the assessment centre also said that they can't send out a duplicate UC50, as it's the DWP that instructs them to do it.

    I can fill the UC50 form in thoroughly within 30 mins, so I'm not overly bothered about having to do it again if necessary.

    That's very quick i must admit. I've just returned my daughters UC50 form and it took me almost 2 weeks to finish it because there was so much information to put. You can also fill it in online and print it yourself.. here. https://www.gov.uk/government/publications/uc50-form-universal-credit-capability-for-work-questionnaire

    I paid the extra to make sure it was delivered the following day. I don't usually do this when returning forms to DWP but as it was sent straight back to the health assessment centre i thought it was worth doing because it's important it's received on time.

    This sort of scenario is exactly why I paid the extra to upgrade the delivery to signed for - peace of mind that it's reached the building at least, even if it takes them time to get to it.  Not that it helps your situation right now of course, but should you need to send anything again it could be worth considering.

    (Extra/upgrading because the standard postage was pre-paid on the envelope, so I only had to pay the difference - thanks to a poster on here mentioning it!)
