Transferring an ISA

I've looked at the website of my current S&S ISA provider (provider A) and they have a ISA Transfer Authority form there to fill in to transfer the ISA to another provider (provider B ).

I've looked at the website of provider B  to where I want to transfer my ISA to. They have the form there at provider B to fill in to state the name of the current provider A in order to transfer the ISA to provider B.

Should I complete only the form at provider B? I'm guessing (since B have more incentive) they would push things along faster than if I fill in the form at provider A?

The responsibility for actioning a transfer lies with the receiving ISA.  So you should contact provider B.  They will then contact your current provider A.  Contacting A yourself may just confuse matters.

The form on provider A's website is for ISA transfers to them, not from them.

ISA transfers must be initiated by the new provider, so you need to fill in the transfer form of provider B.
