To get the cash back credited to your account when the card ceased in November 2007 you must telephone 0870 126 2665 (8am - 9pm 7days) armed with your card number. Enter the options maze until you get to a live agent then request that the money be sent to you. The agent will tell you you will receive your payment cheque within 14 days.:j

If you need to call no need to use the 0870 number, use 0208 1810000 instead, I called, it was answered immediately by Indian call centre who then transferred me to UK customer service agent. Advised that anyone with a credit balance will be sent cheque automatically with the December statement. Obviously if you had an outstanding balance the cashback will be offset against that.
