Yu need to complete the forms and a statement setting out what the specific incidents were and when, and what you are asking the court to order.
If you are applying on notice then the application will need to be served on the person concerned, if you apply without notice you need to say why this is necessary, and be aware that any order made will need to be personally serviced on them so make enquiries in advance about a process server and make sure that you get enough copies of the order and the application to be served. 

IF the application is because of domestic abuse then you may qualify for legal id to allow you to get legal advice. 
Bear in mind that injunctions are intend to provide protection in emergencies, so in any application / statement focus on the most recent incidents which have resulted in you needing the order now - historic incidents may be of relevance if you are claiming that there has been a long pattern of abuse, or to explain why you are scared, if the more recent incidents have been less serious, but in general, you shouldn't spend too long going over lots of historic incidents.
IF you re seeking the order against a partner and are looking or an occupation order as well, make sure that you include in your application if you need that person to continue to make any kind of financial contribution.
