Notice of Disassociation - Questions

Hello, first time poster here. 
I did some searching within the forum and couldn't find an answer to my questions, so would be very grateful if someone can help me please.
I want to fill in applications to the three credit reference agencies to get myself financially disassociated from my ex-spouse.
The problem I have is that I don't have her current address and that is asked for on the forms - can you just write that you don't know? I could supply them with the divorce certificate. 
Secondly, once the disassociation is completed, does she get notified of this? I am trying to avoid any form of communication. 
Any help or advice would be very much appreciated.

Could be no one knows, with apologies.  The credit agencies are tricky to deal with, it may be the best place to ask about address unknown etc, is probably to contact them direct?
How long ago would also be a factor - these things should drop off after X years, however sometimes a trigger links you wrongly again and then it can be there again.

You could also raise a query with the CRA directly and just ask them to dissociate just explain you’ve separated so it needs to be removed they may just do it for you

i am having to do the same, I need to be disassociated from my partner as I do t want my bad credit to affect his great credit, we had a joint account last year but I then removed myself from it but the account is still open but solely in his name but the credit files show we are still active together. I’ve written to transunion and I’m speaking with a customer care support guy who is helping me. Such an annoying thing but needs sorting! 

Trying to speak with anyone on the phone is near on impossible🙈
