How to fill in the R40 tax from ppi claim

I need help - I’ve had ppi payout and paid tax but no longer have the detail how do I fill out the R40 without details ?

You don't.
You need to ask the company that made the payout for the details. Particularly as tax isn't deducted from PPI payouts (they aren't taxable income), only the statutory interest paid alongside the PPI refund.0
