How to contact HMRC for a hard of hearing elderly relative with no internet access

I have been trying to change an address for my relative for more than 2 weeks.
Ideally she wants to fill out a form and return by post. She does not have internet access. There is no postal information on any of the latest HMRC letters. 
I called the HMRC helpline, went through the extensive menus which routed to a different number to call. For that number I went through many menus and at the end the number hangs up. I've been waiting on the HMRC page for web chat for 2 weeks and there has never been an advisor available online at any time of day.
It shouldn't be this difficult for elderly people to get into contact with the HMRC. Can anyone help me with a form or address to change address offline?

    Write to HMRC at this address for all Income Tax queries except complaints.

    You do not need to include a street name, city name or PO box when writing to this address.

    Couriers should use a different address.

    Can you sign them up to have online access to their Personal Tax Account? They can change their address online.
    I realise that they don't have access to the internet, but you do, so you can help them to change their address. 
    You can complain to HMRC, but I doubt it will change things unless lots of other people complain.

    If you just need a form you can contact your MP!
    That's what my friend did as the telephone 'helpline!!' told she could apply online. She doesn't have the internet.
     Local MP office are going to post one to her
    Being polite and pleasant doesn't cost anything!
