    We can’t put a flag over it because it’s too raised, it’s plush with the flags around it. We asked them to lower it and cover it with a flag or to put on a recessed lid on, but they’re saying it’s not their fault.

    You can't just put a manhole wherever you want to, it has to go in the correct access position for the preexisting drains.
    So surely that means if it had to go there, it’s in the wrong place on the plans? The plans show it being in a different area. We get they can’t go anywhere but we’ve asked for it to be covered or have a recessed lid fitted and they’re denying any responsibility.

    Recessed access chamber would be the best result now I think, be persistent.
    Hows the original problem too?

    Inspection chambers have to go either where one drain connects to another or where a drain has to change direction (doesn't happen often, any change in direction is a weak point so best avoided). The plans won't show the exact positioning of the drains, your chamber is within the ballpark measurements of where they would be finally placed. I don't see you have any comeback on this unless your contract specifies that the inspection chamber would be recessed. It's just a cover, not a big deal. You'll get used to it being there. 
    The other thing to consider is the reason why there was a bad smell in the first place. You might have put something down that blocked the drain a bit or it might be one of your neighbours whose drain connects to yours.
