I would never ever pay anyone to fill out a form for me. There's plenty of advice agencies who will help you for free. Although you probably won't be able to get any face to face appointments right now and any help will be through a phone call.Start here with that. https://advicelocal.uk/There's also lots of help available on the internet. Here https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/pip/help-with-your-claim/fill-in-form/There's no specific wording that you should use when filling out any form, just tell it how it is. Add as much relevant information about how your conditions affect you as possible. Then add a couple of recent real world examples of what happened the last time you attempted that actvity for each descriptor that applies to you. These real life examples are the most important thing of all and these are the things they will want to see on any form like this. You may also want to have a read of this https://www.gov.uk/government/publications/personal-independence-payment-assessment-guide-for-assessment-providers/pip-assessment-guide-part-2-the-assessment-criteriaThat link will give you more of an understanding of the PIP descriptors and what they mean, as well as all the crtieria.

In addition to the links poppy has given, this may be useful (summarises some of the important concepts for easy referral)

If you have the capability to complete the form, then I would suggest writing it yourself after researching the links supplied above.
You know how your disability affects you re the PIP activities, so the important thing is to fully explain that on the form. Be aware of the importance of "reliably" in PIP, also what "majority of time" means.
You could start writing a diary.

If you suffer from a specific condition, you may find internet info on PIP relating to that condition. E.g.
