I started my own business on June 1st 2020, due to a job loss before the first lockdown.
I had work, but was not able to fill all commitments due to some customers not allowing me on their property.
I also HAD to self-isolate after coming back from Poland.
Can I have claimed assistance during lockdown & self-isolation?
I have no previous accounts from last year.

You can't really expect any help for Poland, assuming you weren't forced to travel at gun-point.
Even if Poland wasn't on the 'naughty list' when you left the UK, it was pretty clear places were being added and removed from the list on a daily basis, so was always a risk.
