Replacing a lost NHS Optical Voucher

We moved a few years ago, and but my son still attends his yearly check up at the old opticians. Even though its a few miles away, I thought, "It's just once year, and they have his history on file". Anyway, my son had an appointment Sunday just gone and they didn't have the 2nd pair free deal they previously had. But the opticians closest to us does. So I took his NHS Optical Voucher (1 blue form & 1 yellow form). The local optician wern't open till Tuesday. I took him in after school on Tuesday, he chose his 2 pairs of glasses. But when I handed the vouchers over, the yellow form wasn't signed. I planned to get it signed today, but now I can't find it :s 
I know i brought it home, as my wife asked me what his new prescription was and I checked on the form.

If the optician had signed the form, this wouldn't have been an issue  :(
Can anyone advise how I can get an replacement voucher? The issuing option said they can't be replaced, but is this just a trick to get you to pay for another eyetest or something?
Any help would be appreciated? Thanks in advance.
