According to Sainsbury's bank I have died twice

When I applied to Sainsbury's bank for a credit card, I had to contact them, because their credit search companies had flagged up I had died, I spoke to them, and managed to get over this. They seemed satisfied I was still breathing etc, and granted me a credit card, which worked okay for a while, then stopped... Contacting them again, they again said their credit search companies had flagged up I had died, and I told them again I am still breathing, the credit card works again. They advised me to check my credit report, I have done so with the three companies on this site, and none of them flagged up my passing.
How can I find the source of my death? Has anyone else had similar experiences?
NB I have told Sainsbury's I will close my account with them if I have to contact them again regarding my ability to breath etc.

Have you asked Sainsbury's who they are using?
Credit Reference Agencies are big sellers of data and their interests go beyond credit alone. Whilst data can be bought directly from GRO on registered deaths I know a number of life & pension firms use Call Credit to flag any of their insureds/pensioners that have died and triggers their "existence checking" processes (and suspends payments). I've never seen the data that comes from them but from another similar service they give a score of 1-5 with 5 being certainly dead and 1 being certainly alive which deals with the matter that fuzzy logic is used to determine if the registered death is of our Bob Smith born 10/11/1955 or some other guy who shared a name and DoB. It could be you have a namesake that died recently and the algorythms are struggling to work out which one died and which one is still alive.

I wish I could figure out how to trigger it as that kind of mistake is great for compensation and it's always worth having another backup credit card so these things don't cause any hassle.

There are several sources of Deceased lists
the bereavement register - https://www.thebereavementregister.org.uk/

Experian mortality suppressions - https://www.experian.co.uk/business/glossary/mortality-suppressions/
the very fact that there are multiple lists illustrates that there are variations. Quite a lot of companies use more than one source to improve their match rates.
It might be worth contacting each to see which one has the info wrong.

The data probably comes from someone reporting to your file. Some years ago I had an account that was marked as "Closed" and "Deceased." It was in fact not my account at all but one of my late father's which had inexplicably made its way into my file. It didn't actually cause me any problems thankfully so I left it and it eventually fell off.
Probably a good idea to check the status of all open and closed accounts to see if any of them are reporting as deceased.1
