Travelling to Italy

My husband I and would need to go to Rome in a couple of days, but I'm struggling to fill up the self declaration from's last part, at point 8. Do I need to write down the address in Italy I would stay in case I have symptoms? Has anyone travelled to Italy lately? Would appraciate if anyone could help me.

Putting aside the UK requirements a form has to be filled out prior to entering Italy and another one on leaving. It's just basic information. Both were downloaded via a link on the FCO Site.
On my trip to Venice a couple of weeks ago they were just handed into the crew on boarding both ways. What (if anything) happened to them after that is anyone's guess. You could have put "Donald Duck Florida" as they were not scrutinised.0 -
