Do I need to write down the address in Italy I would stay in case I have symptoms? Has anyone travelled to Italy lately? Would appraciate if anyone could help me.

Yes, you need to declare where you will stay in Italy.

Putting aside the UK requirements a form has to be filled out prior to entering Italy and another one on leaving. It's just basic information. Both were downloaded via a link on the FCO Site.

On my trip to Venice a couple of weeks ago they were just handed into the crew on boarding both ways. What (if anything) happened to them after that is anyone's guess. You could have put "Donald Duck Florida" as they were not scrutinised.

I wonder if anyone would be kind enough to forward me a copy of their form filled in as I am really struggling just looking at it.
