SSE low voltage Wayleave claimform. Where?!

Does anyone please have the URL or email address for making backdated wayleave claims on residential property?  I cannot find a claim form page on the SSE DNO website to make my claim directly with SSE. I would rather not use an ambulance chaser company.

I used this when I submitted my claim to SSE.
