Urgent help needed re DHL Proforma labels

Gosh I'm in a muddle.
I have booked a DHL collection for today sending a parcel to Australia.
I have filled in the Proforma and have had to print off 5 Proforma forms.
These need to be stuck to the outside of the parcel preferably in a waterproof docket.
How do I put all 5 A4 Proformas on the parcel?
Do I put them one on top of each other?
Do I put them all in the same waterproof docket?
Do I fold them ?
Writing outside or inside
I'm so confused

I don't know about DHL but many carriers like Fedex and UPS come equipped with self adhesive windows for putting the customs clearance paperwork in.  I'm sure DHL will be the same for international consignments.
