Equifax financial associate

Hi,
I looked at my Equifax report and my partner has been added as a financial associate but there are 2 things I want some help/advice about.
1. He is being listed as a financial associate by Halifax, we have not applied for any credit, we applied for and were refused a halifax DiP.
Is a soft search enough to link people as far as credit file is concerned?
2. His name is incorrectly spelt. Imagine his name was Davies, and they have listed it as Davids. It looks like it has been heard and typed out without confirming which I find very odd.
Should I contact Equifax about having this adjusted?
We are hoping to get a mortgage soon and I dont want to have what looks like multiple financial links risking our chances.
Thank you!

1. Yes
2a. Yes
2b. If Halifax declined the DIP, was it because they spelt his name wrong?

I'd personally be getting in touch with Halifax and ask them to sort this - usually the CRAs can only report the information they are given, so Equifax won't be able to do too much if Halifax are supplying them incorrect info. That said, it probably wouldn't to also put in a dispute with Equifax.
How does your info look on your reports from the other CRAs (Experian/TransUnion)? Are the same errors present?

How do I contact Halifax to adjust this, do I need to complain?
I have not seen it come up on either other report, but the DiP application was done end of july so it hasn't been a full month yet, if that makes any difference.
