Travel Insurance through bank and medical condition declarations

Hi there, hoping someone could help me with travel insurance through banks or annual travel insurance policies. 
If you start taking medication throughout the year for something, are taking tests etc (pre diagnosis), etc... how and when are you supposed to divulge this info to them?
Do you inform them before booking or assume you are covered because you have the service ongoing? 
If you don't want a medical condition covered do you still have to let them know about it and tell them to exclude it from cover?
Can they charge you extra even though you are paying a monthly fee for the insurance already?

Any help welcomed

    You should let them know when the condition presents but insurers don't like undiagnosed conditions very much and so if you have no travel plans then maybe wait until a diagnosis.

    When you inform then they will tell you:
    a) If they will cover it
    b) If there is any additional premium to pay for cover

    Obviously if they will cover it and there is no additional premium to pay you may as well have it covered. If they say they'll cover it for £10,000 and you don't want to pay that much you can decline it and they will exclude it from the policy. There are some things, particularly when not diagnosed yet, that they simply won't offer cover on.

    If you have a holiday booked and insurance in place before you started suffering symptoms of this condition then that holiday should be covered anyway
