Application for forcing sale of flat

Good Morning, Hope someone can help. I am looking to force the sale of my flat, my ex-partner he keeps extending when he wants to sign the papers with the estate agent, each time giving excuses as he is too busy because of work. Now he has promised he will do it in 10 days, as he is away cannot do it until then. I believe its all a delaying tactic as he is currently also in an IVA with payplan.
I've done my research and I know I have to apply for a county court judgement, then an order of sale. But I cannot find the right form! Anyone point me in the right direction to the form number that I need.
Secondly most solicitors in my area don't want to take the sale of the flat on because of his IVA. I have spoken to Payplan if we need to inform them.. but they would not let me know even though I asked them the question in general without disclosing any details.
