Equifax - how do I make an official complaint?

To summarise, last 2 months my Equifax Credit score marked a default with a credit card company, I raised a dispute with them, and it was confirmed it was done in error, and this was then removed, however today the score has updated and I find that Equifax have ;
Removed my home address (still live there)
removed my current account (still active)
removed 2 credit cards (still active)
removed phone contract (still active)
removed car insurance (still active) and,
stated BT (by sport) as dormant
(they have also removed old accounts that have been closed for a while also)
from what I know in experience I would need to raise a dispute and get Equifax to contact the companies to confirm what I am saying, but for all the above? Can someone give some advice on all this as I have never seen this before?

Just raise a dispute over the missing accounts.0

It is 99% likely to be the missing address. It happened to me and once the address was put right, all the accounts showed back up.0
