Urgent - How to correctly sign Deed of c9venant and Licence to Assign forms

Can someone please advise how to sign these documents as we have our witnesses this evening and need to send this to conveyancer in the morning in order to exchange on time but it looks totally confusing as to who signs where and how. Can you please help

the schedule.Where it says 'signed as a deed by the said [ ]', you insert your name between the [brackets] and sign on the rightWhere it says "witness signiture and address" your witness signs and gives their address.DeedWhere it says "signed as a deed and delivered by" you sign to te right.Beneath your signiture and to the right of "in the presence of" your witness signs and adds their address.But I'm surprised you received no instrucions with the forms.2

Sadly our solicitor is totally useless and hasn't provided any info whatsoever. They even forgot to send these documents before, , that's why all the rush now. I only hope everything goes ok from here, so we can finish this and not have to deal with them ever again!
