Does it mean that £5,000 have to be in the account every single day for the whole month?

If you choose that option, yes.
If you choose the other option (make £500 of debit card purchases) then no.

As a reward, you can choose to get a reward (£5 a month) if you keep at least £5,000 in your account at the end of each day.

Other rewards are available.

If you don't meet the conditions of your chosen reward in any particular month, the worst that can happen is that you don't earn the reward for the month in question.
