Over 70's Rates Discount

Hi All,
An over 70's friend of mine who lives alone went to pay their rates last week. She was told at the time that she was eligible for the over 70's discount which is not means-tested and a form was sent out. The form has arrived and this friend has shown it to me, it's looking at very specific information on savings and bank accounts, etc. It doesn't seem right for the rates discount.

Does anyone know if this "benefit form" is the correct form to be completing?
