I'll be as brief as I can: Myself, my wife (Northern Ireland), Mother & Father in-law (Northern Ireland) and Brother and Sister in-law + 2 kids (England) were due to holiday at a house in Italy at the end of July. Due to ongoing uncertainty with 2nd spike of covid-19, we have chosen to cancel the house booking in italy. Party in England were uncertain of being able to drive to Italy and back again. Ryanair have cancelled in-laws flights from Dub - Rome, but ours are still going ahead on an earlier flight. Is it reasonable to think the Travel insurance (renewed yearly with current account Nationwide) will pay out for flights booked last November if we choose not to fly due to those concerns and my wife having Asthma adding to the risk?

Insurance don't pay out on concerns,  it will pay out according to the policy statement. You will most likely need a unfit to fly note from a doctor.
Does your policy cover FCO advice?

What do you mean "but ours are still going ahead on an earlier flight"
What has changed?  the flight number, the time (if so by how much)?

    What is FCO?
We are booked on a 06:20 flight out of Dublin, whereas the in-laws were on an afternoon flight which has been cancelled.

Asthma has been removed as at risk.

Foreign and Commonwealth Office (FCO) they are the ones that offer "advice"  if you go against that advice you won't be insured.
