As you have the letter from the NHS then it's New style ESA that you need to claim and not New style JSA. If you had stated that to start with then i could have advised you this earlier. You need to have paid the correct NI Contributions in tax years April 2019 to March 2017 to claim it. See link. https://www.gov.uk/guidance/new-style-employment-and-support-allowance
