How much tax was paid?

Hi guys,
I received a PPI payout recently from RBS for a £5000 loan taken out in 1994 over 3 years. The letter states the amount of £652.53 includes a refund of the PPI premiums paid plus 8% interest net of any income tax. I want to fill in a R40 form to claim back the tax but  there is no information on how the payment was worked out and I have no idea how much interest was added to the PPI amount. Can anyone help with this? TIA
