Advice required regarding dates on a Codicil being added to a Will

Hello.
After spending the last 2 days on hold waiting to speak to the court of protection and the official solicitor being absolutely no help what so ever, I am reaching out for some guidance (i am a court appointed Guardian for my physically and mentally impaired brother)
to give some context, the Codicil has been authorised by the Court Judge and all i need to do is send it back signed and witnessed, but as they've sent it back again saying the dates are wrong i am at my wits end trying to get what should be a very simple answer to the question - what date?
I have emailed the court (having given up trying to get through by phone - 6 1/2 hours non stop - to the point that i've reached my monthly call allowance on mobile and landline) As time is of the essence and the website says that in 10 working days I may get a response, i am hoping someone can help.
i have attached a redacted copy with notes as to where the dates go, but just to clarify:
1. should the date i put on the Codicil (see note 1 & 2) - be the court order to execute a statutory codicil (19th February)?
2. Can the date I put (when it was signed by me) be crossed out, then the court date entered and initialled?
3. 
The witness & i have signed & dated it to twith the date it was actually signed. Should this be the court date authorising it too?
I really am desperate, as the care home (where my brother lives) is in lockdown and so am I and the witness due to medical/age reasons and really need some guidance.

    I would say yes.

    I can't see any other way of doing it.

    3. The witness & i have signed & dated it to twith the date it was actually signed. Should this be the court date authorising it too?

    I would say it was correct to put the date it was actually signed.

    This is a very difficult and frustrating time.

    The only other thing I can think of is to telephone a local solicitor specialising in wills and trusts 

    https://www.step.org/step-directory for a "telephone conference" - perhaps you could email him a copy of the form, get his advice and pay his fee by faster payment?
