PPI Tax Reclaim form help

Hi I had a couple of successful PPI Claims reclaimed over two financial years, However a little confused as to what to put down on the R40 form.
AS Martin confirms on box 2 you put the P60/P45 details Box 3 is for the PPI received right? then Box 3.1 is for the tax taken off.
Now is this the page called Uk Employment income?
Do I add the amount I earned that year to the PPI payout and do the same for the tax?
Any help would be great?
