Changing Minor's middle name by Deed Poll - Advice needed please

For reasons we have decided we wish to change our son's middle name. He is 3 months old and we are currently completing the forms from Gov UK website. I've put some time aside today to complete this but now need some advice and their office is closed. I was just wondering if anyone is able to help in the meantime (if not i'll call them on Monday).
So when completing the forms there is LOC024 Statutory Declaration for a Minor. This needs to be completed by A person who has known you for no less than 10 years. For a minor it then states they must; Declare how long they have known the person with parental responsibility, which must be for no less than 10 years, and also how long they have known the Minor.
My husband and I have only been together 8 years. I have someone who has said they will be the declarant for this form however they've known me for 12 years but my husband for approx 7 years. So i'm assuming as i'm completing the form I am the person with Parental responsibility. My husband and I have completed form LOC022 and both signed as parents.
I have a Commissioner of Oaths to complete the legal parts of the documents. I thought it was possible to change a babys name up to the age of 1 at the Registry office where it was registered, have you enquired about that option - perhaps I am wrong but worth looking into as baby is so young.

I've had a look online and can't see anything that allows you to do this. You can only reregister if a mistake was made etc. The only options I have are to change by deed poll.0

it states he has to be baptised which my son is not and also this is a different county to where I live. I can't find anything like this on my counties website.0

I can't find anything like this on my counties website.As 74jax says, further down in that section it saysIf the child has not been baptised the parents can complete a form at the Register Office. Once again this process can be done at any time after the birth as long as it can be proved that the change of name took place within 12 months of the birth registration. Once the form has been completed the amended name(s) will be inserted into space 17 of the original birth registration. Certified copies of the amended registration can then be purchased. The full copy certificate will contain both the original name and the new name in space 17. The short version certificate will only record the child's amended forename. From 1st November there is a statutory fee for this service of £40.I suggest you check with your local registrar. If you can get the original birth certificate amended with the new name as described above it's probably going to be easier for the child later on in terns of proving id etc.0

You can re register a child under 12 monthsLife is like a bath, the longer you are in it the more wrinkly you become.0
