How to word a complaint letter help please

I hope I'm posting on the correct board- apologies if not.
I want to write a complaint about a safety issue but not sure how to word it. The story in a nutshell:
Bought a hat for a baby from a major high street chain, the hat had 2 plaits (to keep baby's ears warm), mother nipped to loo, came back to find baby's mouth full of the wool from the hat and baby coughing. Took it back to the shop where the other plait came apart in the assistant's hand. Baby is only 9 months old so hat is a major hazard as baby could have choked.
What do I write?

Have you thought of contacting trading standards as this could be a nation wide problem regards to health and safety

No, but thanks for that. Shoud I contact trading standards instead of the shop or both?
